CoverCheck ("we," "us," or "our") operates doesinsurancecover.com. This Privacy Policy explains what information we collect, how we use it, who we share it with, and what rights you have over it.

We collect minimal data. We do not sell your personal information to third parties. We do not store sensitive health or medical records.

2. Information We Collect

Information you provide directly:

  • Email address — when you submit a form to receive a PA template, appeal guide, clinic list, or HSA eligibility check
  • Insurance provider selection — to generate your coverage estimate
  • Treatment selection — to generate your coverage estimate
  • Clinical situation selection (e.g. BMI range, diagnosis category) — to generate your coverage estimate
  • State of residence — to apply state mandate adjustments to your estimate

Information collected automatically:

  • IP address and approximate location (city/region level)
  • Browser type, operating system, and device type
  • Pages visited, time on page, and referral source
  • Clicks on referral links

What we do NOT collect:

  • Your full name (unless voluntarily provided)
  • Your actual insurance plan details or member ID
  • Your Social Security Number or financial account information
  • Precise diagnosis or medical records
  • Payment information (we do not process payments)

3. How We Use Your Information

We use the information we collect to:

  • Generate and display your personalized coverage estimate
  • Send you the resource you requested (PA template, appeal guide, etc.) via email
  • Send follow-up emails relevant to your coverage situation (you may opt out at any time)
  • Improve the accuracy of our coverage estimates over time
  • Analyze site usage and optimize the user experience
  • Detect and prevent fraud or abuse
  • Comply with legal obligations

We do not use your information for automated decision-making that produces legal or similarly significant effects.

4. Email Communications

When you submit your email address, you consent to receive the specific resource you requested and related follow-up emails from CoverCheck. These may include information about coverage options, cash-pay alternatives, and relevant updates.

Every email we send includes an unsubscribe link. You may opt out at any time. We honor unsubscribe requests within 10 business days.

We use Sequenzy to manage email delivery. Your email address and associated tags (treatment type, coverage outcome) are stored in Sequenzy's systems subject to their privacy policy.

5. Referral Links and Third-Party Providers

When you click a referral link to a third-party provider (such as Hims, Ro Body, Found, Evolve Telemed, AgeMD, or Marek Health), we may receive compensation. We also pass limited information via URL parameters (such as source identifier) to track referral attribution.

Once you leave our site and interact with a third-party provider, that provider's own privacy policy governs the collection and use of your information. We are not responsible for the privacy practices of any third party.

Third-party providers we refer to may be subject to HIPAA if they provide clinical services. We are not a HIPAA covered entity and do not transmit protected health information.

6. Cookies and Tracking

We use cookies and similar tracking technologies to:

  • Remember your session state (e.g. which step of the coverage checker you completed)
  • Analyze traffic patterns via Google Analytics or similar tools
  • Track referral link clicks for affiliate attribution

You can control cookies through your browser settings. Disabling cookies may affect the functionality of the coverage checker.

We do not use cookies for targeted advertising or cross-site behavioral tracking.

7. Data Sharing

We do not sell your personal information. We share data only in the following limited circumstances:

  • Service providers: We share data with tools that help us operate the service (Sequenzy for email, analytics providers, hosting infrastructure). These providers are contractually prohibited from using your data for their own purposes.
  • Referral partners: We pass attribution identifiers (not personal data) to affiliate partners when you click referral links.
  • Legal requirements: We may disclose information if required by law, court order, or governmental authority.
  • Business transfers: In the event of a merger, acquisition, or sale of assets, your information may be transferred. We will notify you via email or prominent notice on the site before your information becomes subject to a different privacy policy.

8. Data Retention

We retain email addresses and associated tags for as long as you are subscribed to our communications. Upon unsubscribe, we remove you from active send lists within 10 business days. We may retain anonymized aggregate data indefinitely for analytics purposes.

Coverage checker inputs (insurance, treatment, situation, state) are not stored server-side. They exist only in your browser session and are used only to generate your real-time estimate.

9. Your Rights

Depending on your jurisdiction, you may have the following rights regarding your personal information:

  • Access — request a copy of the personal data we hold about you
  • Correction — request correction of inaccurate data
  • Deletion — request deletion of your personal data
  • Portability — request your data in a machine-readable format
  • Opt-out of sale — we do not sell data, so this right is already honored
  • Opt-out of email — unsubscribe link in every email

California residents (CCPA): You have the right to know what personal information is collected, to delete it, and to opt out of its sale (we do not sell it). To exercise your rights, contact us at the email below.

EEA/UK residents (GDPR): Our legal basis for processing your email is your explicit consent at the point of form submission. You have the right to withdraw consent at any time by unsubscribing. You also have the right to lodge a complaint with your local supervisory authority.

10. Children's Privacy

The Service is not directed to children under the age of 13. We do not knowingly collect personal information from children under 13. If we become aware that a child under 13 has provided us with personal information, we will delete it immediately.
